School-Based Health Care Coordination: A Sustainable and Scalable Model
School-Based Health Alliance PROJECTAbout This Project
Summary
With multi-year funding from Pivotal Ventures & DHHS Office of Minority Health, SBHA implemented comprehensive care coordination in 53 school-based health centers (SBHCs) serving 30K children in urban & rural marginalized communities in GA, IL, NM, TX, WV & FL. They receive fragmented care, which disrupts their health & education. We built local & state networks, advocated for reimbursement, hired & trained dedicated care coordinators & used technology to deliver integrated, equitable care.
Goals & Objectives
Maximize health, wellness, & academic outcomes for 30K Black, Indigenous People of Color, Hispanic & Appalachian low-income students & families with care coordinators to facilitate integrated, equitable, & cost-effective services. Collect data with integrated tech enhancements & rigorous eval tools.
Results & Impact
Hired 42 SBHC care coordinators in 6 states. Built sustainable care coordination networks of community health centers, school & community partners, youth & families. Increased social determinants of health screening, referral & follow-up. Improved health & education outcomes & reimbursement policy.
